When a healthcare provider (a covered entity) uses LifeHealth or engages with integrated services that require access to protected health information (PHI), and such access is delegated to a third party in order to carry out a function or activity on behalf of the provider, that third party is considered a business associate under HIPAA.

In such cases, LifeHealth requires a Business Associate Agreement (BAA) to be in place between the provider and the business associate to ensure proper handling, security, and confidentiality of health information. This requirement extends to any subcontractors or secondary service providers who may also need access to such data to perform their duties.

Disclosure of Information to Third Parties

LifeHealth does not sell personal information to third parties. However, there are limited circumstances where information may need to be shared:

LifeHealth retains personal information for as long as necessary to provide Services, manage accounts, support clinical care, fulfill legal and regulatory obligations, resolve disputes, and enforce our agreements.

Once all applicable retention periods have expired, we will delete or destroy personal information in a manner designed to ensure that the data cannot be reconstructed or read. If complete deletion is not possible (e.g., due to technical constraints), we will continue to protect the information in accordance with this Privacy Policy.
